- Current Nursing License or Certificate
- Current CPR Card (Expired? Click here)
- Current ACLS / PALS (if required in your area of expertise) (Expired? Click here)
- Current MMR titer or Rubella and Rubeola Titer
- Varicella Titer (history not acceptable)
- Current TB Test or Chest X-Ray and TB Mask Fit (done at time of TB test)
- Current Physical with No Job Restrictions
- Copy of Drivers License or another state issued ID
- Copy of Social Security Card or other acceptable I-9 Document (see I-9 instructions)
- Completed Application (@criticalnursing.com)

- Completed W-4 (Federal Tax document) (@criticalnursing.com)
- AZ-4 (State Tax document) (@criticalnursing.com)
- Completed annual competencies testing (Contact CNS Recruiter)

- Picture ID (Contact CNS Recruiter)
- Completed Drug Screen (Contact CNS Recruiter)
- Completed Background Investigation (Contact CNS Recruiter)
